1. Assist clients with personal care needs such as bathing, grooming, toileting, and dressing
3. Help with meal preparation, light housekeeping
5. Qualifications:
7. Must be 18 years or older
9. Minimum 2 years of home care experience preferred
11. Must have reliable transportation
13. Ability to read, write, and communicate effectively in English
15. Bilingual (English/Spanish) a plus

5. Monitor client safety and report any changes in condition
7. Accurately document visit notes using EVV
9. Follow care plans developed by our care coordination team
11. Provide companionship and emotional support
13. Adhere to infection control and safety protocols
15. Arrive on time and communicate clearly with the office and clients
